Participant Release

I, (we) despite all reasonable precautions implemented for safety, am (are) fully aware of and appreciate the risks, including the risk of catastrophic injury, paralysis and even death, as well as other damages and losses associated with participation in the programs or activities. I (we) knowingly and willingly assume all such risks. Consequently, I (we) hereby for myself, heirs, executors and administrators, do waive and release any and all rights and claims for damages against the owner, operators, coaches and other members of Mile High Dive Club LLC (the release) from personal injury or accident of any sort or nature suffered by me (us), the undersigned, by reason of participation or membership in classes, lessons or any programs or activities of Mile High Dive Club LLC.

I (we) hereby assign and grant Mile High Dive Club LLC the right and permission to copyright and/or publish photographic portraits or video of my diver or myself and/or photographs or videos in which I may be included whole, in part or composite. The above photographs or videos may be used for advertising, trade, art, and/or any other lawful purpose. I hereby waive my right to inspect and/or approve the finished product used in connection therewith. I hereby release and discharge Mile High Dive Club LLC, its successors and all persons acting under the Agency’s permission or authority or those for whom Mile High Dive Club LLC is acting from any liability by virtue of alteration or use in composite form that may occur in any process in completion of the finished product. In addition, I waive all claims for compensation or damages.

Medical Release

I, the parent/legal guardian of the diver for whom I am registering (“Participant”), do hereby authorize and consent to Mile High Dive Club LLC (“Authorized Party”), obtaining for the Participant any x-ray examination, anesthetic, medical or surgical diagnosis or treatment and hospital or emergency room care facility (“Medical Facility”) care to be rendered to the participant under the general or special supervision of any member of the medical staff and emergency room staff licensed under the provisions of the Medicine Practice Act or a dentist licensed under the provisions of the Dental Practice Act and on the staff of any acute general hospital holding a current license to operate a hospital from the State of Wyoming Department of Public Health. It is understood that this authorization is given in advance of any specific diagnosis, treatment, or Medical Facility care being required and, except as expressly limited below, is given to provide authority and power to render care which a Physician and Surgeon or Dentist in the exercise of his best judgment may deem advisable. It is understood that effort shall be made to the above treatment will not be withheld if the parent/guardian cannot be reached. If the Authorized Party is a corporation, this authorization shall include any officer, director or employee of said corporation or its affiliates. It is further understood that I (we) are responsible for all charges for the above mentioned diagnosis, treatment or hospital care.
